What are the risks of an ESR test?

The risks of an ESR test are those of a routine blood draw: brief discomfort, a small bruise and occasionally feeling faint. Infection at the needle site is rare, and the ESR is often measured from a tube taken at the same time as other blood tests.

What does a high ESR mean?

A high ESR means there are more inflammatory proteins in the blood, but it does not show where the inflammation is or what is causing it. Infection, autoimmune conditions, anaemia, pregnancy, kidney disease and, less often, some cancers can all raise it, so a doctor combines it with your symptoms and other tests.

Can a normal ESR rule out a problem?

A normal ESR does not rule out every problem. Some infections and inflammatory conditions, especially early or localised ones, leave the ESR unchanged, so a doctor may still arrange further tests if your symptoms suggest them.

What is the difference between an ESR and a CRP test?

An ESR and a CRP both reflect inflammation, but CRP rises and falls within days while the ESR changes slowly and stays raised for longer. CRP is often more useful for a sudden illness, and the ESR for following a longer-standing condition.

Does age affect the ESR?

Age affects the ESR, because the settling rate rises naturally as people get older and is higher in women than in men. Your doctor judges your result against what is expected for your age and sex, which is why a number that is high for one person can be ordinary for another.

Do I need to fast before an ESR test?

You do not need to fast before an ESR test. Fasting is only needed if another test on the same sample requires it, in which case you will be told when the test is arranged.

How often is the ESR repeated when monitoring a condition?

How often the ESR is repeated depends on the condition and the treatment, and your doctor sets the interval. It is usually repeated after a change in treatment to see whether inflammation is settling, and less often once the condition is stable.
