What is a neck lump assessment?
A neck lump assessment is a planned series of checks that finds out what a lump, or another lasting symptom in the head and neck, actually is. The neck holds lymph glands, salivary glands, the thyroid, muscles and blood vessels, and a swelling can come from any of them. Most turn out to be glands reacting to a recent throat, tooth or skin infection, or a harmless cyst. The purpose of the assessment is to reach a clear answer quickly, so a harmless lump can be left alone with confidence and a serious one is found early.

Who needs it
Book an assessment for any of these warning signs:
- A lump in the neck that is still there after a few weeks, especially if it is firm, painless or growing

Certain habits raise the chance that a symptom needs a closer look. These include smoking cigarettes, shisha or midwakh, regular alcohol use, and chewing betel nut, paan or gutka, a long-standing custom for many residents of the UAE with South Asian roots. HPV, a common virus passed on through close contact, is an increasingly common cause of throat cancers, including in younger adults and non-smokers. Mention any of these openly; the questions are routine and help your surgeon judge urgency.
What happens
Your ENT surgeon starts with your history and a careful examination of the neck, mouth, tongue, ears and nose, feeling each area of glands in turn. A thin flexible scope passed through the nose then gives a direct view of the back of the nose, the throat and the voice box. Most neck lumps are then imaged with ultrasound, which shows whether the lump is solid or fluid-filled and which structure it belongs to. If a sample is needed, a fine needle is guided into the lump under ultrasound, as set out on the ultrasound guided biopsy page. A CT scan or MRI scan is added when your surgeon needs a wider view of deeper structures.
If the results show cancer, your surgeon explains the findings in person and plans treatment with you. Preparing for it
Note when you first noticed the lump or symptom, whether it has changed in size, and any recent infections, dental work or insect bites. Bring a list of your medicines, including any blood thinners, since these matter if a needle sample is planned. Bring previous scan reports or images on a disc if you have them.
For an insured assessment, some plans in the UAE ask for pre-approval before ultrasound, biopsy or CT, so bring your insurance card and Emirates ID. Your first appointment usually needs no fasting.
Afterwards
Your surgeon reviews every result with you and agrees a plan: reassurance and discharge, a repeat check after a set interval, treatment of a cause such as an infection, or surgery. Needle sample results usually take some days, and your team tells you how you will receive them. A family member is welcome at the results appointment.
After a needle biopsy, mild soreness and a small bruise are common. Call the hospital if the biopsy site becomes hot, swollen or increasingly painful, or if you develop a fever. Go to an emergency department straight away for difficulty breathing, noisy breathing, inability to swallow your saliva, or bleeding from the mouth or throat that continues.

What are the risks of a neck lump assessment?
The risks of a neck lump assessment are small and mostly relate to the needle biopsy, which can cause bruising, soreness and, rarely, bleeding or infection at the site. The scope examination can trigger a brief sneeze, gag or nosebleed. A needle sample sometimes gives an unclear result, in which case a repeat sample or a different test is needed.
Does a painless lump mean it is harmless?
A painless lump is not automatically harmless, because a lump from an infection is often tender while a lump from a more serious cause is often painless. Pain or its absence is one clue among several, so a persistent lump needs examination either way.
Does shisha or midwakh carry the same risk as cigarettes?
Shisha and midwakh both expose the mouth, throat and lungs to tobacco smoke and are linked to the same head and neck cancers as cigarettes. A single shisha session can deliver as much smoke as many cigarettes, so regular use is an important risk to mention at your assessment.
Should I take antibiotics for a neck lump first?
Antibiotics help only when a lump is caused by a bacterial infection, and your doctor may try a short course when an infection is likely. If the lump is still there after the course finishes, the next step is ENT assessment rather than further antibiotics.
How long should hoarseness last before it is checked?
Hoarseness lasting more than three weeks should be examined by an ENT surgeon, particularly if you smoke or use shisha or midwakh. The scope examination gives a direct view of the vocal cords and usually an answer at the same visit.
Can a dentist spot mouth cancer?
Dentists often notice mouth ulcers or red and white patches during a routine check and advise an ENT assessment. Any mouth ulcer or patch that stays unhealed for three weeks should be examined, whether you or your dentist spots it first.
