What is a semen wash for IUI?
A semen wash for IUI, also called sperm preparation, is the laboratory step that turns a fresh semen sample into a small, clean suspension of moving sperm. Intrauterine insemination places sperm directly inside the womb, close to the time of ovulation, so they start nearer to the egg.
Raw semen is unsuitable for placing in the womb. Seminal fluid contains natural substances, including prostaglandins, that would cause strong cramping inside the uterus, along with dead sperm, cells and debris. The wash removes these. The laboratory then collects the actively swimming sperm and suspends them in a sterile culture medium designed to keep them healthy until insemination. The result is a small volume, easy for the clinician to pass through the cervix with a fine, soft catheter, containing the sperm best able to reach the egg.

Who needs it
A semen wash is arranged on clinician referral for couples whose fertility plan includes IUI. Couples usually reach this point after an infertility consultation with the gynaecology team and a recent semen analysis. Common reasons for IUI include:
- a mild reduction in sperm count or movement, found at a male infertility assessment with the urology team;
- ovulation problems being treated with fertility medicines;
- difficulty with intercourse or ejaculation;
- unexplained infertility after both partners have been assessed.
For IUI, at least one fallopian tube needs to be open, so this is checked during the fertility assessment.
What happens
The semen wash happens on the day of insemination, and the timing is set by the woman's cycle monitoring or ovulation trigger. The fertility team gives the couple an arrival time.
- Identity check. Both partners confirm their identity with the laboratory, and the container is labelled with both names. Careful matching of sample to couple is a central safety step.
- Collection. The male partner provides the sample by masturbation in a private room at the hospital, into the sterile container.
- Liquefaction. The sample rests briefly so the semen becomes liquid.
- Preparation. The laboratory uses standard sperm preparation methods to separate the moving sperm from the seminal fluid, then rinses them and concentrates them in a small volume of culture medium.
- Assessment and handover. The prepared sample is checked for count and movement, recorded, and handed to the clinician for the insemination, with the couple's details confirmed again.
Preparation takes some time, so the couple may wait at the hospital between collection and insemination.
Preparing for it
Preparation is similar to a semen analysis, and your clinician gives exact instructions:
- Abstinence. Avoid ejaculation for a short period of a few days before the insemination day, as the laboratory advises.
- Documents. Bring Emirates ID for both partners, your insurance card and the referral from your clinician.
- Collection. Use only the container provided. Lubricants, saliva and ordinary condoms harm sperm.
- Health. Tell the team about any fever, illness or new medicines in the weeks before.
- Ramadan. If the insemination falls in Ramadan, discuss the timing of collection and fasting with your fertility clinician in advance.
Arrive on time, because the prepared sample is used close to ovulation.
Afterwards
The male partner can leave once the sample is collected, or stay to support his wife through the insemination. The woman can usually return to normal daily activities after a short rest, following her clinician's advice. The laboratory records the count and movement of the prepared sample, and your clinician may share this information with you.
A pregnancy test is usually arranged about two weeks later, often as a beta hCG blood test.
After insemination, contact the hospital if the woman develops fever, worsening pelvic pain or an unusual discharge. **Go to the emergency department straight away for severe abdominal pain, heavy bleeding or fainting.**

Are there any risks with a semen wash for IUI?
The semen wash itself carries no physical risk, because it is a laboratory process on the sample. The insemination that follows has small risks, including cramping, spotting and, rarely, infection, and fertility medicines used alongside it can lead to twins or more.

A semen wash does not change the quality of individual sperm. It selects the moving sperm and concentrates them, so more of them are placed close to the egg at the right time.
