What is a scrotal Doppler ultrasound?
A scrotal Doppler ultrasound is an ultrasound scan of the scrotum that combines two views: a picture of the testicles, the epididymis behind each testicle and the surrounding tissues, and a colour map of the blood flowing through them. The picture shows shape and texture. The Doppler layer shows whether blood is reaching each testicle, and whether it is flowing more or less than usual.
A detail most men find reassuring: the scan compares the two sides with each other. Each testicle acts as the reference for the other, so a difference in size, texture or blood flow between the sides stands out clearly.

Who needs it
Your doctor may request a scrotal Doppler ultrasound to look into:
- A lump in or around a testicle, to show whether it sits inside the testicle itself or in the structures beside it, which guides the next step
- Swelling of the scrotum, such as a fluid collection (hydrocele) or a cyst on the epididymis
- Pain or tenderness, where increased blood flow points towards inflammation or infection of the epididymis or testicle
- A suspected varicocele, the enlarged, twisting veins above a testicle often likened to a bag of worms
- Male fertility assessment, where a varicocele or a difference in testicle size is relevant
- Follow-up after an injury, an infection or treatment for a varicocele or hydrocele

Sudden, severe pain in a testicle is an emergency: go to the emergency department straight away, because testicular torsion, where the testicle twists and cuts off its own blood supply, needs surgery within hours, and treatment must never wait for a routine scan appointment.
What happens
The scan takes place in a private room. You lie on your back on a couch and lower your trousers and underwear. A towel is placed across your upper thighs and under the scrotum to support it, and another covers your groin, so only the scrotum is exposed. Warm gel is applied, and the probe is moved gently over each side.
Each testicle is examined in turn from top to bottom and side to side, then both are viewed together for comparison. Colour flow is switched on to map the blood supply to each testicle and the epididymis. The probe touches the skin lightly, and the person scanning adjusts the pressure if an area is tender.
For a varicocele, two extra steps are added. You are asked to take a breath and bear down, as if straining, which makes enlarged veins swell and shows blood flowing backwards in them. Part of the scan may also be done while you stand, because the veins fill more when you are upright. The groin may be scanned too, to look at the path of the veins and check for a hernia.
You can ask questions throughout, and the images are reviewed and reported by the radiologist.
Preparing for it
A scrotal Doppler ultrasound needs no special preparation. You can eat, drink and take your usual medicines. Shower as normal beforehand and wear loose, comfortable clothing that is easy to lower.
Bring your referral, a list of your medicines and any earlier scan reports, particularly if you had a scan elsewhere, because comparison with an earlier scan shows whether anything has changed. Note when your symptoms started and whether they began suddenly or gradually. If you would like a male member of staff or a chaperone present, ask when you book.
Afterwards
You can go back to your normal activities, including work and driving, straight away. Wipe away the gel with the tissues provided. The radiologist sends the report to the doctor who requested the scan, who explains the result and any next step, such as treatment for an infection, a follow-up scan, or a urology consultation about varicocele surgery or other treatment.
If your scan was for a painful infection, keep taking any treatment you were prescribed. Supportive, snug underwear and rest often ease scrotal discomfort, and in the Sharjah heat, drinking enough water helps you stay comfortable.
Contact the hospital if the pain or swelling gets worse, you develop a fever, or the skin of the scrotum becomes red and hot. Go to the emergency department straight away for sudden severe testicular pain, pain with vomiting, or a testicle that is suddenly swollen, raised or lying at an unusual angle.

What are the risks and limits of a scrotal Doppler ultrasound?
A scrotal Doppler ultrasound has no known risks, because it uses sound waves and no radiation. Its limits are that the scan shows the structure and blood flow of a lump but cannot always say exactly what the lump is, so blood tests, a follow-up scan or surgery may still be needed. In suspected torsion, surgeons may operate without a scan, because the delay matters more than the image.
Is a scrotal ultrasound painful?
A scrotal ultrasound is usually painless, with only light pressure from the probe. If the area is tender from an infection or injury, tell the person scanning you so they can adjust the pressure.

Can an ultrasound tell if a testicular lump is cancer?
An ultrasound shows whether a lump lies inside or outside the testicle and what it looks like, which tells your doctor how likely it is to need further treatment. Most lumps outside the testicle are benign. A lump inside the testicle is examined further with blood tests and a urology assessment.
Why do I need to strain during the scan?
Straining, or bearing down, raises the pressure in the abdomen and makes enlarged veins above the testicle swell and show backward flow. This is how a scrotal Doppler scan confirms and grades a varicocele.
Can I choose a male member of staff for the scan?
